Assessment Agreement

Library Loaner Assessment Tools 
This will be for members only. You will able to borrow assessment products purchased by staslha for one week with the option for renewal for up to 4 weeks. There will be an agreement that you will have to fill out and then you will have to pick up the test and go! The procedure,  and agreement are above so click on the link to obtain more information.The first test available is the Clinical Assessment of Articulation and Phonology

     Use order form on this site and email  librarian for directions.

  Librarian will email order form if needed.

     Return the order form  (include check for deposit fee made out to STASLHA)

     Librarian will send the materials (including return address label for your convenience)

     Return materials in 30 days (insurance is recommended)

     Upon receipt of materials in good condition within 30 days, deposit fee check will be returned to you

 Material donations or suggestions for library purchases are always very welcome.  Please contact librarian to make request and arrange a pick-up for any materials.
